Forms IQ Privacy Policy

Forms IQ is an AI-powered form creation solution for Microsoft Teams and Microsoft 365. It allows you to create, edit, publish, and manage forms within the Microsoft ecosystem, with responses stored securely in your Microsoft 365 environment.
Information Forms IQ Collects and Stores
Forms IQ does not collect any personal information for its own purposes, nor does it send any personal data to any external services outside of Microsoft's cloud that is required to provide the functionality you request.
- Authentication & access tokens: Your Microsoft 365 credentials are never seen by us. Authentication is handled by Microsoft. Any access tokens or refresh tokens issued by Microsoft to enable Forms IQ to function are stored locally on your device or within the host application's secure storage and are used solely to access Microsoft services on your behalf.
- Form data and responses: All form data, including form definitions, responses, and associated metadata, are stored within your Microsoft 365 tenant using Microsoft Graph APIs and SharePoint. Forms IQ does not operate a separate backend to store your form content or responses.
- Configuration data: If applicable, tenant- or service-specific endpoints and your app preferences (e.g., last selected form, publishing settings, or view preferences) are stored locally to improve your experience.
- AI processing: When you use AI features to generate forms, your prompts are processed by Microsoft's AI services (such as Azure OpenAI) within your Microsoft 365 environment. Forms IQ does not store or process AI prompts outside of Microsoft's infrastructure.
- No analytics or tracking: Forms IQ does not include third-party analytics, advertising SDKs, or trackers.
Use of Your Information
Stored configuration data and, where applicable, access tokens are used exclusively to: establish a secure connection to Microsoft 365 services; synchronize your form data during online mode; enable AI-powered form generation; manage form publishing and sharing; and maintain your selected preferences across sessions. Forms IQ does not build user profiles or share your data for marketing purposes.
Security of Data
Forms IQ is network-capable and connects to Microsoft 365 services via secure web protocols. All local data is protected by the host platform's secure storage mechanisms. Where an application datastore is used, it is encrypted using an application key that is randomly generated at first run and protected by your device or account security (e.g., system keychain, OS-level encryption, or password/PIN/biometric safeguards). Form data and responses are secured by Microsoft's enterprise-grade security infrastructure within your Microsoft 365 tenant.
Data Retention
Forms IQ retains only locally stored configuration data and tokens necessary for operation. Form data and responses are retained according to your organization's Microsoft 365 data retention policies. You can delete local configuration data at any time by signing out and/or uninstalling the app or add-in, which removes local storage and requires fresh authentication the next time you sign in.
Data Sharing and Transfers
Forms IQ does not sell, rent, or trade personal data. Data flows only between your device, Microsoft 365 services, and your specified form recipients when you choose to publish or share forms. Form responses are shared only with users you explicitly grant access to within your organization. We do not operate a separate backend that stores your Microsoft 365 content or form data.
Children's Privacy
Forms IQ is intended for business and productivity use within Microsoft 365. It is not directed to children and does not knowingly collect personal information from children.
Changes to This Privacy Policy
We may update this Privacy Policy from time to time. Any changes will be posted on this page, and the "Effective date" above will be updated.
